Sunday June 8, 2008 According to Abby Sale on Ballad-L,

As I sat down one evening within a small cafe,
A 6-foot, seven waitress to me these words did say:

is the original opening of “The Frozen Logger” — rather than

“A forty-year-old waitress.”

A bit of googling turned up the original at a website devoted to the Grateful Dead, from James Stevens’ “Bunk Shanty Ballads and Tales” 1949:

As I set down one evening in a timber town cafe
A six foot-seven waitress, to me these words did say
“I see you are a logger and not a common bum
For no one but a logger stirs his coffee with his thumb
